Step-by-Step Guide: How to Report an Issue on Mobile


Step 1 —

Open the SimplChek Mobile App

Ensure you are logged into your assigned workspace and operational environment.

Step 2 —Open the Navigation Menu
From the navigation menu, select: Issues
The Issues module allows users to:
  • Create new issues
  • Track reported issues
  • Monitor issue status
  • Review escalations
  • Manage corrective actions

Step 3 —Tap “Create Issue”
On top of the Issues module, tap: Create Issue
This opens the mobile issue reporting form.

Step 4 —Enter Issue Information
Complete all required issue details carefully and accurately.
Required Fields Include:

a. Issue Type Users may select categories such as:

  • Safety
  • Compliance
  • Maintenance
  • Environmental
  • Operational
  • Security


b. Issue Title
Example:
  • Oil Leakage Near Generator Room
  • Missing Fire Extinguisher
  • Failed Safety Inspection
  • Damaged Warehouse Door

c. Description
Include:
  • What happened
  • Exact location
  • Observed risks
  • Operational impact
  • Immediate concerns
Example:“Large oil spill observed beside Generator Unit 3 during inspection. Area presents slip hazard and possible equipment malfunction risk.”

d. Priority LevelSelect the appropriate severity or priority level.
Typical severity levels may include:
  • Low
  • Medium
  • High
  • Critical
Severity helps management prioritize operational response and escalation workflows.

Step 5 —Verify GPS & Timestamp Information
SimplChek may automatically capture:
  • GPS location
  • Date and time
  • Geo-tagged evidence
This helps validate field reporting authenticity and improve operational traceability.

Step 6 —Review the Issue Details
Before submission, verify:
  • Issue title is accurate
  • Description is complete
  • Severity level is correct
  • Evidence is attached properly
  • Required fields are completed

Step 7 —Add Supporting Evidence
Attach evidence to improve issue documentation and investigation accuracy.
Supported Evidence Types:
  • Photos
  • Videos
  • Documents
  • Inspection screenshots
  • Audio notes (if enabled)

Step 8 —
Tap “Report Issue
”Once all information is completed, tap: Report Issue
The issue will immediately be:
  • Logged into the SimplChek system
  • Assigned a tracking reference
  • Visible to supervisors and assigned teams
  • Added to operational dashboards and reports

What Happens After SubmissionAfter submission:
  • Notifications may be sent to responsible teams
  • Escalation workflows may begin automatically
  • Corrective actions may be assigned
  • Supervisors can review and monitor progress
  • Users can track issue status updates
