The Issues module allows users to report hazards, incidents, non-conformances, maintenance problems, and other operational concerns that require attention and follow-up.

Issues are typically created when a user identifies a problem during:-      Task execution

Step-by-Step Guide: How to Report an Issue
Step 1: Open the Issues Module
  1. From the Dashboard, tap Issues.
  2. The Issues Panel will open, displaying:
    • All Issues
    • Open Issues
    • Resolved Issues

Step 2: Create a New Issue
  1. Tap the ➕ Plus icon located in the upper-right corner of the Issues panel.
  2. The Report Issue Form will open.
 
Step 3: Enter Issue Details
Complete the required fields, including:
  • Issue Title
  • Issue Type (e.g., Hazard, Safety, Quality, Maintenance, Compliance)
  • Priority Level (Low, Medium, High, or Critical)
  • Site or Location
  • Due Date (if applicable)
  • Description of the Issue
  • Comments or Observations
Provide clear and accurate information so the issue can be investigated and resolved effectively.

Step 4: Attach Evidence (If Available)
If the form allows attachments:
  1. Tap Upload Image or Add Attachment.
  2. Select an existing photo or capture a new one.
  3. Confirm the upload.
Adding evidence helps reviewers understand the problem more quickly.

 

Step 5: Submit the Issue
After reviewing the information:
  1. Tap Report Issue.
  2. The issue will be saved and added to the Open Issues list for tracking and follow-up.

  • Use a clear and specific issue title.
  • Select the correct Issue Type and Priority.
  • Include enough detail for someone else to understand the problem.

  • Using vague titles such as Problem or Issue Found
  • Selecting the wrong priority level
